PHOENIX (AZFamily) — A woman is dead and two children were taken to the hospital after a crash in west Phoenix late Monday night.

Phoenix police officers and firefighters responded to the three-vehicle crash around 11:30 p.m. at 43rd and Purdue avenues, north of Dunlap Avenue.
