Critical violation: Upon inspection, an upright single door cooler is found to have a digital temperature display of 54 degrees. The following foods were found at these respective temperatures. According to the employee in charge, these foods have been in the cooler longer than two hours: (27) 6-ounce containers of yogurt (52-55 degrees), one half quart 2% milk (55 degrees), 4 lbs. thick chunky salsa (54 degrees), and 2 cups plain yogurt (53 degrees). The cooler is not operating in order to keep TCS (potentially hazardous) foods at 45 degrees or below as required. (Corrected — above foods voluntarily discarded and cooler taken out of service. Maintenance department has been advised.)
Violations: Poly cutting board found with abundant cut marks.
Dishwasher final rinse temperature found at 160 degrees maximum and not 180 degrees minimum as required.
Meat slicer blade guard and back of the blade is found not clean with dried food residue. The interior of the cook line plate warmer not clean with food residue. Black bus pan and several clear plastic pans holding clean utensils and service were found to contain food crumbs and debris. The interior of two stainless steel bulk food containers not clean with food debris. Several wire racks in the walk-in cooler found not clean with a whitish deposit which is likely mildew.
Some lower shelving not clean.
Some floor areas under movable equipment on the cook line not clean.
Current food service permit not posted.
Mitsuba Hibachi & Sushi

Critical violations: Raw, whole-shell eggs (4 cases) noted stacked and stored next to bottled beverages in the walk-in beverage cooler behind bar. (Corrected — cases of raw, whole-shell eggs reorganized and stored only on lower shelf and not next to any bottled beverages.)
Chopped garlic in oil (1 small metal bin) 87 degrees noted stored at cook line (and not kept refrigerated) for an undetermined amount of time. (Corrected — small metal bin of garlic in oil discarded.)
Preparation top/reach-in cooler unit across from the cook line was noted not working properly at time of inspection, and not able to maintain potentially hazardous (TCS) foods at/below 45 degrees during cold storage. Potentially hazardous (TCS) food items were noted stored above 45 degrees in the preparation top portion of the cooler as follows: One small plastic bin raw scallops (51 degrees), two steaks of raw beef (52 degrees), one small plastic bin of cut raw chicken pieces (53 degrees), one small plastic bin raw shrimp (52 degrees), and one dozen raw, whole-shell eggs in metal bin (54degrees.) Potentially hazardous food items noted stored above 45 degrees in the lower reach-in portion of the cooler as follows: 18 wrapped portions prepared noodles (50 degrees), 21 packages commercially prepared dumplings containing pork, 12 dumplings each (48 degrees), and two 4 lb. wrapped portions raw beef. Chef stated the cooler was fully stocked approximately one hour prior between 1:00 pm and 1:30 pm, and were therefore stored above 45 degrees for less than 1 hour. The stationary thermometer located in the back left corner of the reach-in portion of the cooler indicated the internal air temperature of the cooler was approximately 55 degrees. (Corrected — The chef moved all potentially hazardous or TCS foods to the walk-in cooler and adjusted the temperature settings on the preparation top/reach-in cooler unit. An additional stationary thermometer was placed in a more visible place within the reach-in cooler to allow for the internal air temperature to be more easily monitored. Operator placed a container of water in the lower reach-in portion of the cooler and one in the preparation top portion of the cooler to simulate food product temperatures. If the cooler cannot maintain internal air temperatures and food product temperatures of 45 degrees or below at all times, then the cooler must be serviced/repaired. If lowering the temperature settings results in internal air temperatures and food products temperatures of 45 degrees or below for a period of at least 24 hours then the cooler may be restocked.)
Violations: Small plates of raw fish noted stored directly on top of a bin of raw scallops with the bottom of the small plates in direct contact with the raw scallops; containers stacked without a lid or hard barrier between. Container of broccoli noted stacked on top of whole tomatoes in the walk-in cooler without a lid or hard barrier between.
In-use knives stored in crevice between preparation top cooler and adjacent steam table. (Corrected — knives moved to dish machine area to be cleaned, sanitized, and properly stored.)
One sliding door on sushi cooler case noted cracked and repaired with tape; surface noted not smooth and easily cleanable at time of inspection.
Wet wiping cloths noted stored on counter surfaces. (Corrected — wet wiping cloths moved to sanitizer buckets.)
Personal coat noted hanging on food storage rack. (Corrected — personal coat moved.)
Establishment lacked a choking poster at time of inspection. (Corrected — additional choking poster provided at time of inspection.) Allergen notice not posted in kitchen and printed menus lacked the required allergen prompt.
Comments: Discussed with operator the following topics: cold holding requirements, proper cooling techniques and best practices, and visibility/accessibility of stationary thermometers. Recommended periodic monitoring of food product temperatures during cold holding and hot holding.
