Spoiled/moldy food in walk-in cooler area: Lebanon County restaurant inspections, April 21, 2025

Foltz’s Pretzels, 35 S. 8th St. (market) Lebanon, April 11. Pass. The handwash sink in the facility was blocked by personal items and not accessible at all times for employee use. Items were removed.

American Legion Post #559, 35 S. Manheim St. Annville, April 11. Pass. Shredded cheese and gallon of lemonade Food observed in the walk-in cooler area, spoiled/moldy and is adulterated. Discarded. 2 door reach-in cooler on the cook line is leaking water and has standing water and needs to be repaired. Interior flap of ice machine in back room area is cream color flap is broken and needs to be repaired.

Shawna’s Boujee Bakes, 35 S. 8th St. Lebanon, Opening, April 11. Pass. Facility has no sanitizer for washing equipment or utensils in the facility. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. The handwashing sink in the facility has no towel dispenser, no soap and no trash can. Each of these items shall be available.

Black Diamond Restaurant, LLC, 607 Cumberland St. Lebanon, April 10. Pass. The interior of the microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Cleaned immediately. Raw chicken was stored above other items in the walk-in cooler. Corrected. Toilet tissue was not on the dispenser.

El Jibarito Cafeteria & Bakery, 625 Quentin Rd. Lebanon, April 10. Fail. The handwashing sink in the front service area has no hot water, no towels and no soap. Raw chicken was stored above Ready to Eat food in one reach-in refrigeration unit; specifically, the raw chicken was on the top shelf over open pitchers of juice. Picture is attached to this report. The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. A wooden shelf above the prep area and a shelf beneath a prep table containing gallon containers of ingredients were not clean. No dates were observed on any item in the facility. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. The facility needs to designate one of the existing sinks to be the handwashing sink for the rear prep area and the warewashing area. The sink shall be conveniently accessible to both of these tasks. One container of food equipment and utensils was not clean.

M&B Pizzeria Retaurant, LLC, 701 Lehman St. Lebanon, Follow-up, April 9. Pass. One set of shelves in the walk-in cooler is rusted and can no longer be properly cleaned. The rear door located in the prep area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Rodent droppings were observed under a prep table in this area. Paper towel dispenser empty at the handwash sink in the toilet room by the rear prep area. Corrected. Toilet tissue is not on the dispenser. This means that each person is touching the inside of the roll of toilet tissue. A light fixture above the hot food service area has dirt in it from a previous water leak. It has not been cleaned since the incident. One spray bottle, a working container in prep area, used for storing chemicals/cleaners taken from bulk supplies, was not marked with the common name of the chemical. Lighting is inadequate in the room behind the main food prep room; one light fixture is not working.

Speedway #06768, 302 E. Evergreen Rd. Lebanon, April 9. Pass. Commercially processed sausage open pack was marked beyond the 7 days. Prep date was for 4/5 and discard date was 4/26. Corrected. Non-food contact surface (Milkshake machine arm) not cleaned at a frequency to preclude accumulation of dirt and soil.

Cookies Cupcakes and More LLC, 157 E. Market St. Jonestown, April 8. Pass. Interior top and sides of microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected. Flooring under prep table of the food facility is extremely dirty, dusty, and in need of cleaning.

Frog’s Hollow Tavern, 2271 State Route 72, Jonestown, Regular/Complaint, April 8. Pass. Interior top of microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Cleaned Interior of ice machine at bar area with pink- matter around ice making area and in need of cleaning. Machine turned off and is in process of cleaning. Hood Filters with accumulation of grease and dust and in need of cleaning. Last Hood cleaning was completed as of Oct. 2023. One door reach-in cooler on the cook-line with standing water and wet rags stored on the bottom. Units needs to be repaired of leaking water. Custom butcher beef (marked not for sale) products such as liver, chuck roast, round roast, shoulder roast, sirloin steak, and hamburger, is stored in the walk-in freezer with restaurant food and not labeled for personal consumption (use). Corrected. Flooring under equipment on the cook-line with grease and food debris and in need of cleaning. Observed in-use knives and /or cleavers stored between table edges or between tables, an area not easily cleanable & sanitized. The food facility does not maintain Food Employee Certification records as required. At time of inspection no person was on duty who maintains Food Manager Certificate.

Perserverance Fire Co., 22 S. King St. Jonestown, April 8. Pass. Clean knives stored in plastic container wit excessive debris. Corrected. Blue ice scoop bucket at the bar area is cracked and with black-matter on the bottom. Corrected, discarded. Hood filters in with grease and dust accumulation. Hood cleaners are scheduled to come out. Counter mounted can opener, a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected.

Italian Oven On Wheels, mobile food facility Type 4, 110 W. Maple Ave. Myerstown, April 7. Pass. cutting board stained.

The Manor Cafe @ Freeman Building, 1 Boyd St. Cornwall, April 7. Pass. No signage at self service counter of apples stating to was prior to consumption. Cabinets in front counter area at the self service area with excessive debris and needs cleaned. Observed wet wiping cloths on cook-line area, not being stored in sanitizer solution. An open employee’s beverage (can of Monster) container was observed on cook-line area, a food preparation area. Corrected. Caulking around the debris sink in the dish area with black-matter and needs to be cleaned and or replaced.

Pucillo’s Pizza & Subs, 52 W. Main St. Myerstown, Change of Owner, April 7. Pass. No violations.

Wendy’s # 19215, 2420 W. Cumberland St. Lebanon, April 7. Pass. Hanging grease and dust on hood filters on fryers. Time in lieu of temperature being used as a control for potentially hazardous foods, but Cut tomato and lettuce foods observed being held for more than 4 hours. Food was discarded. Facility is not placing new sticker on product when removing from walk-in to the line. Hood filters above the fry area are excessively greasy and dusty, hanging grease and dust over fryers. Floors, walls, and ceilings throughout the kitchen facility are greasy, dusty, grimy all around dirty. Under and behind all equipment with accumulation of grease. Kitchen area needs deep cleaning. Two separate spray bottles of triple play store with food or above food in the kitchen area. Corrected.
