Also, it was found that facility is storing mop buckets, brooms and mops on shelving outside of the building. To prevent vector harborage and contamination, store these in the designated mop sink area.

Ensure all scoops have handles and the handles are kept out of the food product. Personal ice cream from the day before was left in the freezer next to the service area. Ensure all personal food items are kept in a designated area, away from food intended for customers. Manager removed it immediately. In use spray bottles of degreaser and bleach were not labeled with name of the chemical. Make sure all chemicals are properly identified with the common name. Correct immediately.

Guthriesville Dairy Queen, 1225 Horseshoe Pike, Downingtown, March 20. Pass. Food dispensing utensil observed in the ice bin and the dry candy storage containers stored in the food and not with handle above the top of the food in the container. Remove the scoops immediately and ensure to store the scoop not in the containers or in the containers with the handle up and out of the food items. Softer serve boxes were observed to the floor and not 6 inches above. Move boxes onto shelving immediately. A refrigerator that is not in use is in the retail area. Employee noted that management is deciding to get it fixed or remove it. Ensure this refrigerator is either removed or fixed. Ehsan fried mixer herbs can was removed off the shelf and corrected on site. The following shelving is made of raw wood, and is not durable, smooth, non-porous, non-absorbent; Raw wood and ply wood has been installed to provide additional storage space above the food prep room and protein grinder. Please paint this wood with a high gloss paint in efforts to make smooth, easily cleanable, and non-absorbent. Two coats maybe required.

Tel Hai Retirement Community (Food), 4300 Tel Hai Circle, Honey Brook, March 20. Pass. A couple Bain Maries were observed with food debris in the top area where the lid rests when opened. It was reported that areas would be cleaned immediately after lunch service. The floor grout between tiles in the food prep area of the Main Health Care kitchen has become in disrepair in some areas making it difficult to clean. Re-grout areas needed within 60 days. The floor in the dishwashing room of the Main Health Care kitchen was observed to be pitted. Plans must be made to resurface floor within 60 days.

The Artillery Brewing Company, 333 Granite Alley, West Chester, March 20. Fail. The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. Employees must wash hands before putting gloves on to complete tasks. Istructed employee to do so during inspection. Comply daily. Pitas in the kitchen area stored open with no covering. Items were covered by staff. All products must be properly wrapped and stored. Food ingredient storage containers, in the facility, are not labeled with the common name of the food. Label all items. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the multiple refrigeration units, is not being date marked. All food must be labeled and dated with a 7-day shelf life. Product in question was voluntarily discarded. Comply everyday. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following areas: – inside of microwave – shelving in kitchen area – interior of all refrigeration – floors in kitchen area – floors in brewing area (observed grains behind kettles). The handwash sink located in the bar area does not have water at a temperature of at least 85 degrees F. There was no hot water. It was later stated that the water gets too hot so an employee turned off the hot water. Hot water was then turned on. Either install a mixing valve to control hot water temperature or turn on cold water so hands can be washed between tasks. The handwash sink in the kitchen area was not accessible at all times for employee use. Observed a hand sink in disrepair and not function because of various plumbing issues. Repair immediately as the only other hand sink is not close enough to the food preparation area. Brewery door near office door area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Repair door and work with pest professional to seal all gaps to prevent contamination. A Restricted use pesticide is being applied in the food facility by a non-certified applicator. Found multiple bait traps and insecticide sprays throughout the facility. Person in charge stated the facility has a certified pest control company but was unable to provide documentation at time of inspection. All pest control chemicals were voluntarily discarded. Provide this information at next inspection.

Walgreen’s #10388, 494 Nutt Rd. Phoenixville, March 20. Pass. The following need further cleaning and sanitizing: Interior bottom shelf of 2 door reach in cooler in back ware house area; Interior bottom shelf of 2 door reach in freezer in back ware house area; Bottom shelf under Dairy section of wall retail coolers. Floor in between table leg and wheel castor behind front service counter unclean.

Fultoo Biryani House, 8 E. Welsh Pool Rd. Exton, March 19. Fail. Scoops without handles are observed being used as a food dispensing utensil. This is not allowed. Obtain a food dispensing utensil that has a handle and keep the handle up and out of the food items. The following foods were left out of refrigeration: Hardboiled eggs (50 degrees F). Both food items were voluntarily discarded on site. Time in lieu of temperature being used in the food facility to control ready to eat Time/Temperature Controlled for Safety (TCS) foods (Biryani Rice) without written procedures or documentation to verify disposition of food Ensure this food item’s time tracking is maintained and records can be provided. The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was >200ppm, rather than 50-100 ppm as required. Drain the sink and ensure the sanitizer concentration is 50-100ppm. Sanitizer basin of the 3 compartment sink was observed to have food scraps. Ensure this basin is clear of any food scraps. Clean immediately before resetting the sanitizer. The following non-food contact surfaces need to be cleaned: Underneath all the dry storage shelves in the basement; Ice on the floor of the freezer; Wall around the 3 compartment sink; Sweep and mop around/underneath the 3 compartment sink, the spice shelving, and the small stoves. Clean immediately. The handwash sink in the kitchen area was blocked by a shelf and not accessible at all times for employee use. Move the shelf so that the handsink is accessible at all times. Move shelf within 24 hours. The following fixtures were observed to be leaking: Handwash sink in the warewashing; Faucet between the rinse and sanitize basin of the 3 compartment sink. Contact a plumber to repair within 2 weeks. Plastic equipment wrap was observed on the hood in the kitchen. Remove this wrap immediately to ensure there is no cross contamination. Door into the basement from the outside has a gap at the bottom and does not protect against the entry of insects, rodents, and other animals. Install a door sweep to prevent against any unwanted entry. Working container was not labeled with the chemical that was inside. Label bottle immediately.

Lorenzo & Sons, Pizza, 27 N. High St. West Chester, Follow-up, March 19. Pass. Commercially processed ready to eat food, located in the Pepsi cooler, and held more than 24 hours, is not being marked with the date it was opened. Found bottled sauces with not date marking. All products must have common name and date marking noting a 7 day shelf life. All products must be date marked. Comply daily.

Residence Inn Phila, 10 N. Pottstown Pike, Exton, March 19. Fail. Eggs were being stored above other food items in fridge. Eggs should be stored at the bottom to avoid any cross contamination. Employee moved them immediately. Handwash sink and mop sink are being used as the same sink. Facility should stop using these sinks as the same. Please relocate handwashing to the sink next to the ovens. Handwash sink had a bucket inside of it at the time of inspection. Please make sure handwash sinks are clean and available for use at all times. Employee moved these immediately. The following plumbing fixtures were observed to be leaking: 3 compartment sink faucet between the rinse and sanitize basin; The water filter system below the sink next to the ovens. Work order in place. Contact maintenance and have these leaks repaired within 2 weeks. The following fixtures need to be repaired: Drain stopper on the rinse basin of the 3-compartment sink does not hold water. The drawer underneath the coffee brewer is water stained and damaged. Replace/fix these fixtures within 2 weeks. Current CCHD food manager is no longer working at this establishment. Current manager is signed up to take a test. Within 90 days, ensure a certificate is received and you apply to Chester County for CCHD CFM certificate. Link will be sent.

Santino’s Tap & Table/Ram’s Head Bar and Grill, 40 E. Market St. West Chester, Follow-up, March 19. Pass. Loose rubber door gaskets observed on the Superior cooling unit in upstairs bar. Replace or repair. The food facility does not maintain Certified Food Manager records as required. Have Serv Safe certified manager apply for Chester County Certified Food Manager.

Saucey’s Pizza, 12 E. Market St. West Chester, Follow-up, March 19. Pass. No violations.

Taco Picoso, 1455 Baltimore Pike, Toughkenamon, March 19. Fail. Containers of food were stacked on top of each other in the refrigerator without solid lids. If pans are stacked they must have solid lids. Do not store food in grocery bags. Use food grade bags only. To go containers of salsa were not kept in the refrigerator or on ice. These were found in the trailer and at the BBQ area. All salsas must be held at 41 degrees F or less. Discard all salsa out of refrigeration and keep the refrigerator going forward. Handsink was blocked and was not accessible for handwashing. Do not store anything in front of or in this sink. Install screens in the door and serving windows. Door may not be kept open unless there is a screen. Service windows must be closed or screened when not serving customers.

The Adyar Cafe, 327 E. Lincoln Highway, Exton, March 19. Fail. A food employee was observed touching cooked pasta – a ready to eat food – with bare hands. Pasta was discarded during the inspection. Foods in the prep tops and cooler in front server area stored open with no covering. Foods were covered during the inspection. Food ingredient storage containers (squeeze bottles at cook line and dry goods in back storage area) were not labeled with the common name of the food. Containers were labeled during the inspection. Food dispensing utensil in dry goods and sauces in cook line right-side prep top observed stored in the food and not with handle above the top of the food and the container. Utensils were removed during the inspection. Replace with scoop with handle. Observed food boxes stored directly on the floor upstairs rather than 6 inches off of the floor as required. Move foods to be 6 inches off of the floor as required. Sauce and chutney was held at 49 degrees F, in the server area, rather than 41 degrees F or below as required. Foods were discarded. Repair/adjust unit to maintain foods at 41 degrees F or below as required. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the prep tops and front server area cooler, are not being date marked. Begin practice immediately. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration at the 3 compartment sink and mechanical dishwasher. Obtain quaternary ammonium and chlorine test strips. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Chlorine bottle was empty. Bottle was replaced and machine measured at 100ppm. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following: Floors under equipment in the kitchen; Interior of the microwave at the cookline; Exterior of grinder and mixer person in charge stated are currently not in use. Steps leading upstairs have gaps on the side and are not a smooth and easily cleanable surface. Close gaps. Observed 4 cots and cushions in the upstairs of the food facility indicating use of the food facility as living or sleeping quarters. Remove immediately. Exit door in kitchen area of the food facility is being propped open. Door was closed during the inspection. Exit door in kitchen has a gap at the bottom and does not protect against the entry of insects, rodents, and other animals. Close gap. Paper towels were not available at the handwashing sink at the cook line. Paper towels were supplied during the inspection. Ceiling tiles missing in the mop sink area and need replaced. Stain ceiling tiles observed in female restroom. Ensure there are no leaks and replace ceiling tiles. Two working containers in the warewashing area, used for storing cleaners taken from bulk supplies, were not marked with the common name of the chemical. Bottles were labeled during the inspection. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. Post.

Barnaby’s, 15 S. High St. West Chester, March 18. Fail. Observed bagged ice stored directly on the floor in walk in freezer area, rather than 6 inches off of the floor as required. Store bagged ice on a rack off the ground. Various juices were held at 43 degrees F, in the side bar area, rather than 41 degrees F or below as required. Product was voluntarily discarded. Use ice bath or alternate location to store opened TCS juices until unit is repaired. Food facility has a reminder statement on the menu for foods that are/or could be served raw or undercooked to the consumer; however, does not clearly disclose which foods the reminder statement applies to. Must note all foods that might be served undercooked. The sides and base of the charbroiler are being wrapped in aluminum foil. Cardboard lining shelving in basement. Cook line prep top was storing foods in the bottom at 48 degrees F and foods at the top at 44 degrees F rather than 41 degrees F or below as required. Foods stored overnight were discarded. Repair/adjust/monitor. Unit must maintain foods at 41 degrees F or below as required. Commercially processed ready to eat food (soy sauce), located at the cook line, and held more than 24 hours, is not being marked with the date it was opened. Begin practice. Observed rusted utensils stored at cook line. Utensils were discarded during the inspection. Observed cracked wood bowl being used as a scoop/measuring cup for foods in the prep top. Bowl was removed during the inspection. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean restroom in kitchen. Clean behind faucet and repair seal connecting back handwashing sink to wall. Spray bottle in the warewashing are used for storing a cleaner was not marked with the common name of the chemical. Bottle was labeled during the inspection.

The following surface finishes observed broken and need of repair: Cove base missing to left of back door at floor / wall juncture; Missing wall and floor cove base under counter of handoff plane and by cold brew dispensing. Repair wall and install cove base. Broken / cracked test strip holder above three bay sink. Remove or replace test strip older.

Magnetic knife strip, a food contact surface, was observed to have food residue and was not clean to sight and touch. Strip was cleaned and sanitized during the inspection. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer.
