Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for March 5, 2025:

- Cottage cheese and egg salad kept past discard dates

3 violations
- No policy for clean-up of vomit
- Raw chicken stored above beef
- Lentil soup not kept hot enough
The Pitic Restaurant – 1580 E Pima St, Phoenix, AZ 85034
4 violations
- Green and black growth on stew
- Ground beef stored over vegetables
- Salsa Verde not date marked
- Not enough sanitizer in dishwasher
Ono Hawaiian BBQ – 4972 S Power Rd, Mesa, AZ 85212
8 major violations
- No certified food manager on duty
- Cook handling raw meat then not washing hands
- Another employee handling raw chicken then not washing hands
- Hand wash sink blocked by trash can
- Food not separated properly in fridge
- Cutting boards with accumulation of food debris
- Stored utensils with food debris
- Chicken and beef not held at proper temperature
